  Note:  This form is in PDF format and can be partially completed on a computer.  However, it must have a standard hand-signed signature, and be either faxed to DSS at 919-989-5324 or mailed to DSS at P.O. Box 911, Smithfield, NC 27577.  You can also drop the form off at the agency in the DSS Drop Box located in front of the Main DSS building.
