This application for a subdivision evaluation is intended to assist the surveying community by allowing the process to begin without having to make a trip to the Environmental Health Office.

Simply download the application form ; complete it in the entirety, enclose the calculated fees along with blue line copies of the project and send by mail or another form of delivery to:
